What you should bring to your tax preparation appointment

 

🧾 Return taxpayers

Regardless of filing status:
  1. Government-issued photo ID (if requested or asked)
  2. Social Security card or ITIN letter (if requested or asked)
  3. Bank routing & account number (if requested or asked)
  4. IRS letters or notices received during the year (if any)
  5. Form 1095-A (if you had Marketplace health insurance)
  6. Health insurance coverage info (if applicable)

 

🧾 First time visitor: ALL Taxpayers (Required)

Regardless of filing status:
  1. Government-issued photo ID (Driver’s License or Passport)
  2. Social Security card or ITIN letter (taxpayer, spouse, dependents)
  3. Last year’s tax return (if available)
  4. Bank routing & account number (for direct deposit and/or payment)
  5. IRS letters or notices received during the year (if any)
  6. Form 1095-A (if you had Marketplace health insurance)
  7. Health insurance coverage info (if applicable)

Why We Request Identification & Social Security Numbers

To prepare an accurate and compliant tax return, the IRS requires us to verify the identity of every individual listed on the tax return, including the taxpayer, spouse, and any dependents.

Government-issued photo identification and Social Security numbers (or ITINs) help us:
  • Confirm identity and prevent tax fraud or identity theft.
  • Ensure the correct reporting of income, credits, and deductions
  • Verify eligibility for tax benefits such as the Child Tax Credit, Earned Income Credit, and education credits
  • Comply with IRS due-diligence and preparer compliance requirements

All information provided is kept strictly confidential and used only for tax preparation and filing purposes, in accordance with IRS regulations and data-privacy standards.

Personal / Individual

  • Single
  • Married
  • HOH
  • Sole Proprietor
👤 Single (No Dependents)
In addition to the general items:
  • Income Documents
    • W-2 (employment)
    • All the 1099s such as:
      1. 1099-NEC (non-employee compensation)
      2. 1099-MISC (Miscellaneous income)
      3. 1099-G (government-unemployment income)
      4. 1099-DIV (Dividends earned)
      5. 1099-INT (Interest income)
      6. 1099-R (retirement or pension distributions)
      7. 1099-SA (Social Security benefits)
Deductions & Credits (if applicable)
  1. Student loan interest (Form 1098-E)
  2. Tuition statements (Form 1098-T)
  3. IRA contributions
  4. Charitable contributions
  5. Mortgage Interest Statement (if you own a house)
    1. Solar panel contract if you have one or any type of solar contracts.
  6. Medical expenses (if itemizing)
  7. Vehicle contract if you purchase a one in the tax year
💍 Married (Filing Jointly or Separately)
Bring everything listed above, plus:
  1. Spouse’s photo ID
  2. Spouse’s SSN or ITIN
  3. Income documents for both spouses
  4. Prior-year return showing filing status
  5. Any prenuptial or separation agreements (if filing separately)
👨‍👩‍👧 Taxpayers WITH Children or Dependents which are called Head of Household – HOH
In addition to the above:
Dependent Information
  1. Full legal name of each child/dependent
  2. Social Security number or ITIN
  3. Date of birth
  4. Relationship to taxpayer
  5. Proof of residency (school, medical, or daycare records)
Child-Related Tax Forms
  1. Childcare expenses & provider info
    1. Provider name, address, EIN/SSN
  2. Form 1098-T (college students)
  3. Education expenses (books, supplies, tuition)
  4. Adoption records (if applicable)
🧑‍💼 Self-Employed / Schedule C (Sole Proprietor)
If you have a business reported on Schedule C:
Income
    • 1099-NEC / 1099-K
    • Gross receipts (cash, Zelle, Cash App, Venmo, Stripe, PayPal, etc.)
    • Business bank statements
Expenses
  • Mileage log (business miles)
  • Vehicle expenses
  • Office supplies & equipment
  • Advertising & marketing
  • Phone & internet (business portion)
  • Home office info (square footage)
  • Insurance

We only share your information with the tax software company that we use to prepare the return, and if you did not pay our preparation fees up front, we will share your information with the bank that we use to collect our fees, and such bank is a pre-approved bank with the IRS and the tax software company.
